Shape-based preprocessing for the ga_calculation_without_fovea v1 step.

The cache holds RAW model output. This step consumes GA pathology output, so it selects the raw->segmentation pipeline by interrogating the frame's shape (rather than matching on the producing model's ref/version — a mock model with the same output shape resolves identically). Unknown shapes pass through unchanged.
